Work Environment\n

This role is primarily remote, allowing you the flexibility to work from your own office while delivering care to patients at a hospice center. Key Responsibilities\n

• Assess, diagnose, and evaluate the mental health status of patients utilizing clinical tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to identify treatment needs.

• Develop individualized treatment plans that are culturally sensitive and address the diverse needs of the patient population.

• Conduct ongoing assessments of patient progress, implementing interventions designed to support improvements in mental health and well-being.

• Maintain comprehensive and accurate treatment records, including clinical notes and necessary documentation, in compliance with all regulatory and department protocols.

• Collaborate closely with patients, families, and interdisciplinary teams to formulate and monitor effective treatment plans tailored to desired outcomes.

• Coordinates referrals to other treatment providers and serves as a liaison between patients and multiple care resources to optimize support and care.

Compensation and Benefits\n
