Key Responsibilities\n

• Conduct comprehensive assessments to identify social, emotional, and environmental factors affecting clients and their families during hospice care.

• Develop individualized service plans tailored to each client's unique circumstances, including specific goals, intervention strategies, and progress evaluation.

• Provide compassionate counseling, emotional support, and crisis intervention to clients and families facing end-of-life issues.

• Collaborate with clients to create personalized service plans that connect them to appropriate community resources, facilitating access to services such as financial assistance, grief counseling, and palliative care.

• Engage in crisis intervention, proactively assessing risks and developing safety plans to ensure client well-being.

• Work collaboratively with various community agencies and organizations to coordinate services and advocate for clients’ needs, ensuring a holistic approach to care.

• Participate in case conferences and team meetings, contributing insights and sharing best practices to enhance team effectiveness and client outcomes.

Patient Population / Client Focus\n

In this role, you will primarily focus on working with clients facing terminal illness and their families, addressing their emotional and logistical needs during a critical time. You will be instrumental in helping clients navigate their care options and facilitating discussions around advance directives, emotional support, and bereavement resources.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

• Strong understanding of hospice care practices, end-of-life issues, and the experience of grief and loss.

Compensation and Benefits\n
