This position offers a competitive salary ranging from USD 87,000 to 105,000 per year, complemented by a SIGN-ON BONUS.

Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues as a vital team member.

• Conduct thorough patient intakes focusing on diagnostic assessments and functional evaluations, providing valuable recommendations for treatment goals and plans.

• Deliver brief, solution-focused interventions for patients who require immediate mental health support.

• Advise primary care providers on the suitability of patients remaining within the primary care framework or needing referrals to specialized facilities.

• Initiate follow-ups to monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.

• Develop and implement relapse prevention plans tailored to individual patient needs, aiding in their long-term stability and health maintenance.

• Identify at-risk patients and create strategies to prevent the exacerbation of their mental health conditions.

• Manage and coordinate comprehensive behavioral health services, seamlessly connecting patients with primary and external treatment providers, with patient consent.

• Maintain detailed documentation of patient progress and relevant diagnostic information within the treatment chart.

• Engage in proactive communication with primary care providers to discuss patient needs and progress, collaborating on treatment strategies when necessary.

• Encourage behavioral changes in patients at risk for physical disorders, guiding them towards healthier lifestyle choices.

• Empower patients by instilling self-management skills and providing educational resources for their active participation in treatment and recovery.

• Consult with clinic management and fellow team members regarding behavioral services, contributing to program evaluation and outcomes.

• Assist patients in adhering to medical treatments established by primary care providers, aiding them in managing any side effects experienced from medications.

• Adhere to the Code of Ethical Conduct for Clinical Professionals in all aspects of care delivery.

• Perform additional duties as assigned to support the clinic's mission and operational goals.

Patient Population / Client Focus\n

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• A current LPC (Licensed Professional Counselor) license is mandatory.
