Key Responsibilities\n

• Conduct thorough ongoing assessments to effectively address the therapeutic needs of patients, ensuring that care plans are tailored to individual circumstances.

• Facilitate open communication with supervisors, family members, and healthcare staff regarding patient progress, fostering a collaborative and supportive environment.

• Develop tentative schedules to optimize appointments, home visits, and progress monitoring, maintaining flexibility to meet patient needs.

• Carry out essential home visits coordinated for in-home observations, chart reviews, and staff interviews, providing holistic care.

• Oversee the implementation and effectiveness of treatment plans, making adjustments as necessary to ensure positive patient outcomes.

• Actively participate in interdisciplinary team meetings to discuss patient cases and share insights that contribute to comprehensive care.

• Complete detailed monthly and quarterly progress notes documenting patient advancements and required interventions.

• Organize training sessions to ensure that all staff members are competent in program execution and proper documentation practices.

• Provide ongoing support through follow-up interviews and home visits to ensure continued adherence to treatment goals.

• Maintain meticulous records that support data accessibility for teamwork and accountability.

• Act as a liaison with the community and mental health stakeholders, fostering relationships that benefit patient development.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Oregon.
