Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify clients' mental health needs as well as their strengths and coping resources.

• Design and implement individualized treatment plans, adapting interventions that align with each client's unique situation and goals.

• Provide a diverse range of therapeutic modalities, including individual, family, and group therapy, utilizing evidence-based approaches like cognitive-behavioral therapy (CBT) and trauma-informed care.

• Facilitate crisis intervention sessions, executing risk assessments and safety planning when necessary to support client safety.

• Document all client interactions and clinical assessments accurately and in a timely manner to ensure compliance with legal and ethical standards.

• Collaborate with your treatment team to discuss client progress, treatment efficacy, and necessary adjustments to care plans.

• Lead group therapy sessions and psychoeducational workshops to provide clients with tools and techniques for managing their mental health.

• Engage in continuing education to remain current with the latest practices in social work and mental health care.

Patient Population / Client Focus\n

Your work will focus on clients within a hospice setting, comprising both individuals experiencing terminal illness and their families. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required, along with active state licensure as a Licensed Clinical Social Worker (LCSW).
