Key Responsibilities\n

• Utilize your training and expertise to conduct thorough psychosocial assessments and devise individual treatment plans that prioritize the unique needs of each patient.

• Implement evidence-based behavioral interventions, including motivational interviewing and solution-focused techniques, to engage patients effectively and support their treatment goals.

• Actively coordinate with a variety of healthcare professionals, including psychologists, physicians, and other service providers, to maintain an integrated care model facilitating comprehensive patient support.

• Systematically monitor and track patient progress toward established treatment goals through both virtual and in-person follow-ups, ensuring timely adjustments to treatment plans as necessary.

• Assist in the management of psychotropic medications as prescribed, focusing on monitoring for side effects, treatment adherence, and overall efficacy, elevating the standard of care provided to patients.

• Participate in team meetings and supervision sessions to share insights, foster collaboration, and contribute to continuous improvement in our service delivery.

Patient Population / Client Focus\n

This role is primarily focused on patients in hospice care, often dealing with significant life transitions and end-of-life considerations. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Possession of a current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in Florida is essential.

• At least one year of post-graduate experience working independently with clients in a therapeutic capacity.

Compensation and Benefits\n

The salary range for this position is competitive, with a yearly compensation between USD 80,000 and 100,000, commensurate with experience.
