Key Responsibilities\n

• Conduct thorough assessments to develop individualized treatment plans that align with clients’ needs and goals.

• Prepare a detailed schedule for appointments, home visits, and other essential monitoring activities while ensuring compliance with program guidelines.

• Coordinate home visits that include in-home observations, chart reviews, and data collection to provide comprehensive support.

• Implement and oversee program plans effectively, ensuring adherence to established protocols and best practices.

• Participate in interdisciplinary team meetings to discuss client progress and share insights to enhance treatment strategies.

• Complete and maintain accurate documentation, including monthly and quarterly progress notes, to ensure compliance and track outcomes.

• Organize and lead training sessions for staff, enhancing team competence in program implementations and documentation processes.

• Conduct follow-up interviews and home visits to ensure continued support for clients, promoting their engagement in the therapeutic process.

• Act as a liaison within the mental health community, fostering relationships to reach developmental goals that benefit both clients and the clinic.

Qualifications\n
