Key Responsibilities\n

• Perform comprehensive assessments to evaluate patient needs and develop tailored treatment plans.

• Maintain open lines of communication with supervisors, direct care staff, families, and relevant parties to discuss patient progress and outcomes.
\n
• Prepare and submit tentative schedules for appointments, home visits, and necessary observations, facilitating ongoing monitoring of client progress.
\n
• Coordinate in-home visits, conducting thorough chart reviews and staff interviews to gather vital data for treatment analysis.
\n
• Ensure adherence to program plans and treatment protocols, carrying out effective implementation strategies.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing valuable insights and collaborative strategies.
\n
• Document monthly and quarterly progress notes to track client development and reflect on required adjustments to treatment.
\n
• Conduct training sessions to help staff enhance their competencies regarding program implementation and documentation practices.
\n
• Deliver ongoing follow-up sessions to reinforce support and ensure continued progress for individuals.
\n
• Serve as a liaison to the public and mental health community, advocating for support towards developmental goals.
\n
• Assist in devising procedural training materials aimed at enhancing treatment programs for clients.

Patient Population / Client Focus\n

In this role, you will work with a diverse client base that includes both adults and youth, facing various challenges such as family dynamics, trauma, and substance misuse. Your ability to create a safe and supportive environment will be vital in fostering trust and encouraging clients to actively engage in their healing journey.

Qualifications\n

• A minimum of a Bachelor’s Degree in Counseling, Psychology, or a related field.

• Current, active, and unrestricted licensure as a Professional Counselor (LPC) in the state of Colorado.
\n
• At least 2 years of clinical counseling experience is required, demonstrating a strong foundation in mental health treatment.
\n
• Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma is highly desirable.
\n
• In-depth understanding of legal and ethical standards surrounding mental health practices.
\n
• Exceptional communication skills to build rapport with a wide range of clients and collaborate effectively within a healthcare team.

• Proficiency in using Electronic Medical Records (EMR) systems and associated technologies.
\n
