Key Responsibilities\n

• Conduct comprehensive assessments to evaluate patients' mental health needs accurately and develop personalized treatment plans.

• Maintain clear communication with supervisors, healthcare team members, families, and caregivers to discuss individual progress and coordinate care.

• Manage and organize a tentative schedule for appointments and home visits to monitor patient engagement and treatment efficacy.

• Coordinate and execute home visits as necessary for in-depth observations and patient assessments, ensuring adherence to care plans.

• Oversee the implementation of therapy and ensure the treatment plans are executed effectively by all involved parties.

• Participate actively in Interdisciplinary Team Meetings, providing insights and strategies to enhance patient care.

• Document patient progress through monthly and quarterly notes, staying accountable to both the patients and the organization.

• Lead training initiatives to equip staff with the necessary skills and knowledge to implement therapy plans proficiently.

• Offer ongoing support through follow-up interviews, home visits, and observations to sustain patient engagement and address evolving needs.

• Serve as a liaison between patients and the broader mental health community, advocating for resources and support relevant to developmental goals.

Qualifications\n
