Key Responsibilities\n

• Perform ongoing assessments of patients’ mental health needs, crafting treatment plans that are tailored to individual circumstances.

• Maintain clear and consistent communication with family members, guardians, and interdisciplinary team members to discuss patient progress and collaborative strategies for support.

• Develop a structured schedule of appointments, home visits, and supervisory observations to monitor patient outcomes and ensure adherence to treatment protocols.

• Conduct coordinated home visits as needed, engaging in thorough observations and chart reviews to deepen your understanding of clients’ environments.

• Ensure the effective implementation of treatment plans, making adjustments as necessary in response to progress and circumstances.

• Actively participate in interdisciplinary team meetings to discuss cases and contribute to collective decision-making processes.

• Document monthly and quarterly progress notes that accurately reflect patient developments and therapeutic efforts.

• Organize and facilitate training sessions to ensure that staff members are proficient in the application of treatment plans and related documentation.

• Conduct follow-up visits and assessments to ensure ongoing support for patients, promoting sustained improvement in their mental health.

• Serve as a liaison with the community and public health entities to advance our collective goals in mental health outreach and education.

Qualifications\n
