Key Responsibilities\n

• Conduct detailed assessments of patients to tailor treatment plans that address their unique mental health needs.

• Maintain clear communication with supervisors, direct care staff, and family members regarding the progress and well-being of clients.

• Develop a tentative schedule for individual appointments, home visits, and other necessary assessments to ensure comprehensive care.

• Coordinate home visits to conduct evaluations and gather data, providing insightful analysis for treatment adjustments.

• Ensure the effective implementation of care plans, regularly monitoring progress and modifying strategies as needed.

• Actively participate in interdisciplinary team meetings and collaborate with colleagues across various specialties to improve integrated care.

• Document and complete monthly and quarterly progress notes, maintaining compliance with all legal and ethical standards.
\n
• Organize and facilitate training sessions to ensure all staff members are proficient in program implementation and documentation.
\n
• Provide ongoing follow-up interviews, home visits, and monitoring to ensure continued support for patients, adapting strategies as necessary.

• Serve as a liaison with families and the broader mental health community to advocate for client growth and development.

This role involves working with a diverse range of clients, including adults and youth, who require comprehensive mental health support. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of New Jersey.

• Minimum of two years of clinical counseling experience is essential to effectively support our clientele.

• Strong theoretical foundation with expertise in family therapy, couples therapy, substance misuse, or trauma is highly valued.

• Excellent interpersonal skills with a proven ability to build rapport with clients from diverse backgrounds.
