Key Responsibilities\n

• Engage in collaborative care planning with patients and their support teams to develop tailored interventions that address rehabilitation and treatment goals.

• Implement established social work methodologies and integrated patient education programs to assess barriers to mental health and support treatment pathways.

• Provide psychosocial support to patients adjusting to significant health changes, including those undergoing dialysis, to enhance their quality of life.

• Utilize evidence-based techniques to deliver counseling services aligned with your licensure and training.

• Conduct goal-directed educational sessions for patients seeking organ transplants, facilitating their understanding of the transplant process and available resources.

• Help patients navigate community resources, including home health services and vocational rehabilitation, to optimize their treatment outcomes.

• Maintain up-to-date knowledge of local vocational and educational rehabilitation opportunities and guide patients in accessing these services to promote their career and educational goals.

• Assess and facilitate discussions around advance directives, providing necessary resources and coordinating with families to ensure that patients’ wishes are respected.

• Evaluate patient psychosocial status and participate in interdisciplinary assessments to identify strengths and needs that affect overall health and rehabilitation.

Qualifications\n

Compensation and Benefits\n
