Key Responsibilities\n

• Assist patients in navigating community resources, including home health services and vocational rehabilitation, to ensure they achieve optimal treatment outcomes.

• Conduct assessments to identify patients' understanding of advance directives, facilitating necessary discussions with healthcare teams and family members.

• Collaborate with interdisciplinary teams to evaluate patients' psychosocial status, identifying strengths and areas of need that may influence their rehabilitation journeys.

• Participate in the development of individualized care plans, recommending interventions that align with patients' rehabilitation and treatment goals.
