Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate the emotional and practical needs of individuals and families in hospice care.
\n
• Facilitate the development and implementation of personalized care plans that align with the patients' and families' goals and preferences.
\n
• Provide ongoing counseling and emotional support, helping families cope with grief, loss, and the emotional challenges of terminal illnesses.
\n
• Collaborate with healthcare providers, including physicians, nurses, and chaplains, to ensure comprehensive and holistic care for our clients.
\n
• Monitor patient and family progress through regular check-ins and adjust support strategies as needed.
\n
• Advocate for patients’ needs and rights, connecting them to community resources and additional support services when necessary.
\n
• Educate families on the hospice process, including available services and bereavement options.
\n
• Adhere to all relevant Federal and State regulations governing hospice care and social work practice.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals receiving hospice care, as well as their families. Many clients are facing life-limiting conditions and require compassionate support as they navigate emotional, spiritual, and physical challenges. By providing tailored interventions, you will help families make informed decisions during a critical stage of life, contributing to a dignified and supportive transition.

Qualifications\n

• A Master's Degree or Doctorate in Social Work from an accredited program.

• Active LCSW license from the Department of Public Health.
\n
• Valid driver’s license for potential community outreach.
\n
• A minimum of one year of experience in a healthcare setting, particularly with hospice or end-of-life care.

• Strong commitment to ethical practice, patient-centered care, and effective collaboration.

• Exceptional communication, organizational, and time management skills.

• Ability to navigate sensitive topics with empathy and professionalism.

Preferred Experience\n

• Previous experience in hospice or palliative care.
