Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, adolescents, families, and groups to identify their mental health needs and triage appropriate interventions.

• Provide graduate-level clinical social work or counseling services tailored to the unique needs of clients, employing evidence-based practices.

• Engage in regular communication with multidisciplinary teams, participating in patient rounds and documenting treatment plans to ensure seamless coordination of care.

• Demonstrate competency in attending to the physical, psychological, social, educational, and safety needs of patients across diverse age groups.

• Deliver thorough case management services through collaboration with hospital staff, community professionals, and external programs to ensure a comprehensive approach to client welfare.

• Timely and ethically complete documentation that meets departmental standards, ensuring transparent tracking of client progress and treatment outcomes.

• Integrate Cognitive Behavioral Therapy (CBT) and other evidence-based modalities into treatment plans, aiming to support patients and families effectively.

Patient Population / Client Focus\n

The therapist will work with a broad array of clients, including children, adolescents, and adults facing various mental health challenges in an outpatient setting.
