Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through direct interviews, observations, and detailed medical record reviews to identify emotional, social, and environmental factors that affect patient recovery.

• Offer individual and family counseling sessions, focusing on emotional adjustment, coping mechanisms, and mental health support.

• Collaborate with the rehabilitation team to develop comprehensive, individualized treatment plans that address both psychological and physical aspects of patient care.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance programs.

• Provide education and ongoing support to patients and their families regarding community resources and post-rehabilitation strategies to ensure continued success.

• Participate actively in discharge planning and facilitate smooth transitions for patients moving back to home or other care settings.

Patient Population / Client Focus\n

Your work will primarily focus on individuals recovering from various conditions, including strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Working with diverse client backgrounds, you will address their unique emotional and psychosocial concerns as they navigate their rehabilitation journeys, thus directly contributing to their overall health and independence.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
