Work Environment

As a physical therapist in our home care program, you will work onsite directly in the community of Charlottesville. This role requires flexibility and adaptability as you navigate various home environments and tailor treatment plans to meet individual patient needs. Key Responsibilities

• Conduct comprehensive evaluations of patients, including detailed assessments and thorough history reviews, to determine appropriate treatment protocols and document findings clearly.
• Communicate evaluation results and treatment options effectively to patients and their families, ensuring they are actively involved in goal-setting for their rehabilitation journey.
• Demonstrate expertise in executing individualized treatment plans that are responsive to a patient's condition and the latest medical best practices.
• Collaborate with a multidisciplinary team of healthcare professionals as needed, recognizing when to adjust treatment plans based on observable changes in the patient's condition.
• Manage discharge planning and follow-up care efficiently, completing timely and precise documentation of all patient interactions in accordance with regulatory standards.
• Exhibit effective time management skills to meet productivity targets while maintaining a full appointment schedule.
• Partake in departmental goals and initiatives, helping to uphold compliance with accreditation requirements and government regulations.
• Engage in ongoing professional education and training to ensure your skills and knowledge remain current and effective.
• Be proactive in enhancing the patient experience through excellent customer service and flexible care options.
• Participate in community outreach initiatives aimed at improving access to care.

Patient Population / Client Focus

Your primary focus will be on providing care for patients recovering from a variety of conditions, including post-surgical rehabilitation, chronic pain management, and mobility issues. These patients rely on your expertise and empathetic approach to improve their physical function and overall quality of life.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A valid state license for Physical Therapy required for practice.
• Current certification in Basic Life Support (BLS) for Healthcare Providers through the American Heart Association.

Preferred Experience

• While a minimum of two years' experience in a traditional clinical setting is beneficial, new graduates are encouraged to apply as we value fresh perspectives and a commitment to learning.
• Experience with home health care practices is a plus but not mandatory.
