Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through direct interviews, observations, and detailed reviews of medical records to understand the factors impacting patient recovery.

• Provide individual and family counseling, addressing a variety of adjustment issues, emotional concerns, and psychosocial factors that may affect rehabilitation efforts.

• Collaborate closely with a multidisciplinary rehabilitation team to design and implement individualized treatment plans that align with each patient's unique psychosocial needs.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, to ensure post-rehabilitation success.

• Help compile and participate in discharge planning to facilitate seamless transitions for patients moving from care settings back to their homes or other environments.

Patient Population / Client Focus\n

Our patients comprise a diverse range of individuals dealing with varied conditions, including but not limited to neurological disorders, orthopedic surgeries, spine injuries, and post-stroke recovery. Your role will be to support these individuals not only emotionally and socially but also to assist them in practical ways that contribute to their rehabilitation and overall well-being.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

Compensation and Benefits\n
