Key Responsibilities\n

• Engage with physicians and other healthcare professionals to provide updates on patient progress, needs, and concerns, ensuring a coordinated approach to care.

• Maintain meticulous and timely clinical documentation that adheres to departmental policies, including progress notes and treatment plans.

• Collaborate with the healthcare team to develop and refine treatment plans based on comprehensive psychosocial assessments and accepted therapeutic interventions.

• Provide counseling services and crisis intervention for patients, focusing on the assessment and treatment of emotional and behavioral challenges.

• Guide clients through discharge planning by conducting thorough care coordination, facilitating access to necessary regional resources and services.
