Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and financial needs, and collaborate with medical teams to integrate these assessments into comprehensive care plans.

• Develop and deliver individual and group counseling sessions focusing on topics such as adjustment disorders, grief, and coping strategies to empower patients.

• Work in unison with various community organizations to coordinate post-discharge services, ensuring seamless transitions and continuity of care post-treatment.

• Provide critical emotional support and education to patients and their families, presenting treatment options and available resources in a clear, empathetic manner.

• Participate actively in hospital committees to lend expertise on social work practices, improving department protocols and patient care processes.

• Engage in crisis intervention strategies when required, offering immediate support to patients facing acute distress or challenges.

• Design and implement effective discharge plans by connecting patients with appropriate local resources, enhancing their overall recovery and support.

• Maintain compliance with all documentation standards to ensure accurate records that uphold institutional policies and legal requirements.

Patient Population / Client Focus\n

The successful candidate will work primarily with a diverse patient population that may include individuals experiencing mental health disorders, family therapy needs, and those requiring support through challenging life transitions. Qualifications\n

• A Master's degree in Social Work, obtained from an accredited program.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.
