Key Responsibilities\n

• Act as a key liaison between clients, their families, and outpatient providers, ensuring that all parties are informed and engaged in the care process.

• Engage with insurance companies to manage pre-certification processes and ensure appropriate utilization management for mental health services.
\n
• Contribute to discharge and aftercare planning by evaluating client needs and establishing comprehensive follow-up care strategies.
\n
• Provide therapeutic meal support, modeling healthy relationships with food while navigating emotional and psychological aspects of eating.
\n
• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances, incorporating an understanding of their intersecting identities.

Patient Population / Client Focus\n

This role primarily supports clients within the hospice setting who may also be facing mental health challenges related to addiction, grief, or chronic illness. You will work closely with older adults, ensuring their social, emotional, and psychological needs are met through individualized care strategies.

Qualifications\n

• A Master’s degree in Social Work from an accredited university.

• Possession of a valid LICSW license in Washington State.
\n
• Proven experience working with older adults, particularly in the context of hospice care and mental health.

• Strong clinical skills alongside effective diagnostic abilities.

• Exceptional communication and interpersonal skills, with an ability to build rapport quickly.

• Aptitude for working both independently and collaboratively in a remote setting.

Compensation and Benefits\n
