Work Environment

This position is primarily onsite, allowing you to engage directly with patients in their own environments. You will work in an intimate and supportive setting, where you can form lasting relationships with patients and their families. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach efforts, including health fairs on weekends, is also an expected part of your role. This structure ensures that you maintain a work-life balance while contributing meaningfully to community health initiatives.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals across various demographics, ensuring a comprehensive understanding of their physical and mental health needs.
• Diagnose and manage both acute and chronic medical conditions, with a specific focus on mental health disorders, while providing preventive care and promoting health and wellness.
• Collaborate with community organizations and professionals to develop and implement impactful health education programs aimed at enhancing mental health literacy.
• Deliver culturally and linguistically appropriate care, advocating for patients' rights and their access to necessary healthcare services.
• Effectively coordinate referrals to specialists and ensure timely follow-up care, thus enhancing continuity of care for patients.
• Maintain meticulous and accurate medical records in accordance with healthcare regulations, utilizing electronic health record (EHR) systems.
• Engage actively in community outreach and public health events to raise awareness about mental health and facilitate access to services.
• Stay informed on community health trends, evidence-based practices, and relevant legislation to continually enhance care delivery and patient outcomes.

You will serve a diverse patient population in Babylon and the surrounding areas, including those from underserved communities who may face barriers to accessing traditional healthcare. Qualifications

• Possess a current Nurse Practitioner certification.
• Active New York state nursing license is mandatory.
• A Master's degree in nursing from an accredited institution.

Compensation and Benefits
