Key Responsibilities\n

• Conduct ongoing assessments of clients to ensure that individual needs are adequately met, adjusting strategies as necessary to optimize therapeutic outcomes.

• Communicate effectively and compassionately with supervisors, team members, and the families of clients to provide updates on progress and any necessary adjustments to treatment plans.

• Develop and maintain a structured schedule for client appointments, including telehealth sessions and any necessary follow-ups to ensure continuity of care.

• Plan and coordinate essential home visits to accommodate necessary assessments and observations, enhancing the understanding of clients' situations.

• Actively participate in Interdisciplinary Team Meetings to collaborate and contribute to a comprehensive approach to client care.

• Maintain accurate records and documentation of progress notes, ensuring availability of data to all relevant parties.

• Facilitate training for team members and staff on the effective implementation of therapeutic techniques and documentation practices.

• Provide follow-up support through interviews and observations to assist clients in reaching their goals.

• Serve as a liaison to the public mental health community, collaborating on outreach initiatives and advocacy for clients’ mental health development goals.

Qualifications\n

• A Master's or Doctorate degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
