Clinica Recovery LLC is in search of a dedicated and compassionate Physical Therapist (PT) to join our expanding home health care team in Princeton, New Jersey. Work Environment

The selected candidate will work onsite, primarily visiting clients in their homes throughout the Princeton area. Our home care environment is supportive and dynamic, fostering strong relationships with patients and their families. Key Responsibilities

• Conduct thorough in-home evaluations to assess patients' physical abilities and challenges, identifying unique needs and developing comprehensive care plans tailored to individual goals.
• Implement evidence-based therapeutic interventions, including therapeutic exercises, gait training, and balance retraining techniques, to promote physical rehabilitation.
• Educate patients and their caregivers on individualized treatment protocols and home exercise plans, empowering them to undertake active roles in their recovery journeys.
• Collaborate with physicians and other healthcare professionals to align on patient progress, adapting treatment plans based on interprofessional assessments and recommendations.
• Ensure compliance with Medicare regulations and maintain meticulous documentation to track patient progress and service delivery.
• Travel within your assigned territory to reach patients' homes, utilizing effective time management to accommodate scheduling needs and deliver consistent, quality care.

Patient Population / Client Focus

The patients you will work with span a range of ages and abilities, including those recovering from surgery, managing chronic conditions, or rehabilitating from accidents or injuries. Your ability to establish trust and rapport will be essential in supporting their rehabilitation efforts and enhancing their engagement in the recovery process. You will be instrumental in helping clients regain not just physical functionality but also independence within their home environments.

Qualifications

• A valid New Jersey state license to practice as a Physical Therapist is required.
• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Strong communication, organizational, and interpersonal skills, enabling effective collaboration and patient interaction.
• Demonstrated ability to work independently and adaptively in a home care setting.
• Proficient use of technology for documentation and patient management is beneficial.

Preferred Experience

• A minimum of two years of professional experience as a Physical Therapist is preferred.
• Previous experience in a home health setting is highly desirable, giving candidates a distinct advantage.
• Familiarity with Medicare policies and experience in ensuring compliance in documentation are advantageous.

• A competitive annual salary ranging from $110,000 to $125,000, commensurate with experience and qualifications.
