Key Responsibilities\n

• Provide thorough clinical consultations to clinical staff, addressing mental health challenges across a variety of patient demographics, including geriatric populations and individuals facing substance use disorders.

• Accurately interpret and communicate critical information related to patients and their families, including comprehensive social histories, to ensure all team members and providers are well-informed.

• Coordinate and facilitate the development of individual patient care plans, making recommendations based on assessments of home situations and other social determinants of health.

• Plan, implement, and review treatment protocols, collaborating closely with providers to ensure continuity of care and optimal patient outcomes.

• Document and complete progress notes efficiently after each client session, utilizing our electronic health record system to maintain accurate patient records.

• Participate in interdisciplinary team meetings and case reviews as assigned to contribute to the collective expertise of our clinical staff.

Qualifications\n

• A Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Possession of independent licensure in New York State as either a Mental Health Counselor (LMHC), Clinical Social Worker (LCSW), or Marriage and Family Therapist (LMFT) is essential.

• Demonstrated ability to complete clinical documentation such as progress notes efficiently and accurately.
