Key Responsibilities

• Conduct thorough psychosocial assessments to understand clients' mental health issues, strengths, and available resources.
• Deliver individualized, family, and group therapy sessions, employing evidence-based modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed approaches.
• Develop and implement personalized treatment plans, establishing clear goals and interventions for each client.
• Offer crisis intervention services, conducting risk assessments and formulating safety plans as necessary to manage acute mental health situations.
• Facilitate support groups and psychoeducation classes to promote community healing and provide valuable resources for families in hospice care.
• Maintain thorough and timely documentation, adhering to ethical standards and legal requirements in clinical recordkeeping.
• Collaborate actively with the interdisciplinary treatment team to ensure client-centered and coordinated care throughout the therapeutic process.
• Engage in continuous professional development and training to remain current with best practices in social work and therapeutic modalities.

Patient Population / Client Focus

In this role, you will primarily focus on supporting individuals and families navigating the complexities of hospice and palliative care. This includes assisting clients dealing with grief, loss, anxiety, and depression associated with terminal illnesses. Qualifications

• Master's Degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a mental health or outpatient practice, particularly in hospice or end-of-life care settings.
• Strong understanding of evidence-based therapies and a commitment to implementing best practices in clinical care.

Compensation and Benefits
