Work Environment

Key Responsibilities

As a Licensed Clinical Social Worker, you will take on various responsibilities that are crucial to the success of our patient care services:

• Conduct comprehensive evaluations and assessments to develop tailored care plans for patients and their families.
• Participate in the development and implementation of Individualized Family Service Plans (IFSP), monitoring progress to ensure optimal outcomes.
• Build and maintain rapport with children and families, fostering a supportive and nurturing learning environment.
• Utilize evidence-based practices to recommend strategies and interventions aligned with family needs and child development priorities.
• Provide ongoing family support and education regarding developmental milestones and challenges, employing coaching models and natural learning environment practices.
• Ensure compliance with all Federal and State regulations while upholding the highest standards of practice and ethical conduct.

Patient Population / Client Focus

Your primary focus will be on children undergoing hospice care, especially infants and toddlers with disabilities or developmental delays. You will play a crucial role in the early intervention process, helping these young patients and their families navigate complex emotional and practical challenges during a sensitive time in their lives. Understanding their unique needs and creating a nurturing environment is essential for their growth and well-being.

Qualifications

To be successful in this role, candidates must meet the following qualifications:

• A Master's Degree or Doctorate in Social Work is required.
• A current LCSW licensure granted by the Department of Public Health.
• A valid driver's license is necessary for any potential home visits.
• Demonstrated commitment and one year of experience working with young children in early intervention settings.
• Exceptional organization and time management skills to effectively handle multiple cases.
• Strong interpersonal and communication abilities, demonstrating respect and collaboration with team members and clients alike.

Preferred Experience
