Work Environment\n

This remote position allows for greater flexibility while still being integral to our hospice care teams. You will function independently and as part of a multidisciplinary team consisting of psychiatrists, nurses, and case managers, working cohesively to provide comprehensive care and support for patients and their families during sensitive and often challenging times.

Key Responsibilities\n

• Conduct thorough psychosocial assessments that evaluate clients' mental health conditions, strengths, and needs, tailoring interventions to each unique situation.

• Deliver individual, family, and group therapy sessions using a variety of evidence-based therapeutic modalities, such as cognitive-behavioral therapy and trauma-focused therapy.

• Collaborate closely with healthcare providers to develop and implement individualized treatment plans that focus on achievable goals and effective interventions.

• Facilitate group therapy and psychoeducational sessions that empower participants to explore their feelings, develop coping strategies, and strengthen their support systems.

• Provide immediate crisis intervention and risk assessments for patients facing acute challenges, ensuring safety planning is in place as needed.

• Maintain accurate and timely documentation for all client interactions and interventions, adhering strictly to ethical and legal standards of practice.

Our client base primarily consists of individuals facing end-of-life issues, along with their families. Qualifications\n

• Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Kansas.

Compensation and Benefits\n
