Key Responsibilities\n

In your role as a Licensed Clinical Social Worker, the following responsibilities will be integral to your daily operations:

• Develop and implement Individualized Family Service Plans (IFSP) in collaboration with family members and healthcare teams.

• Apply evidence-based practices while adhering to all Federal and State regulations governing social work in hospice care.

• Ensure compliance with safety and health standards to protect both patients and staff.

Patient Population / Client Focus\n

The primary focus of this role is to provide compassionate and effective support to patients receiving hospice care, along with their families. You will engage with individuals facing seriously life-limiting illnesses, assisting them in managing complex emotional, social, and practical needs. By working closely with families, you will help them navigate challenging decisions and provide tools they need for emotional resilience.

Qualifications\n
