Key Responsibilities

• Conduct comprehensive evaluations of patients in a home care setting, assessing their physical capabilities and identifying their rehabilitation needs.
• Develop and implement individualized treatment plans based on evidence-based practices, utilizing techniques such as exercise prescription, hands-on therapy, and the use of therapeutic modalities.
• Monitor patient progress through objective measurements, adjusting treatment plans as necessary to ensure optimal outcomes.
• Educate patients and their families about their conditions, rehabilitation goals, and the importance of adherence to prescribed therapies.
• Collaborate with other healthcare professionals, providing insights into patient care and ensuring continuity of care across different settings.
• Document patient interactions, responses to treatment, and progress in compliance with regulatory and accreditation standards.
• Support the training of patients in the use of assistive devices, fostering independence in their daily activities.
• Engage in continuous professional development through workshops, training sessions, and participation in professional organizations.

Patient Population / Client Focus

The successful candidate will work with a diverse patient population, including children and adults of varying ages and medical diagnoses. This role focuses on treating individuals recovering from a range of conditions, including post-surgical rehabilitation, sports injuries, chronic pain, and neurological disorders. Your compassionate approach will be vital in helping patients achieve their functional goals and improving their overall well-being.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapist license to practice in Florida.
• Strong communication skills, both written and verbal, to effectively interact with patients and their families.
• Proficiency in electronic medical records and documentation practices.

Preferred Experience

• At least 2 years of experience in a clinical setting, though new graduates with strong educational backgrounds will also be considered.
• Experience with rehabilitation practices for diverse patient populations.

Compensation and Benefits
