Work Environment

Located in the beautiful and growing community of Lehigh Acres, Florida, our rehabilitation facility provides a welcoming and professional environment that promotes teamwork and collaboration among healthcare providers. Our standard working hours are Monday through Friday, from 8:00 AM to 5:00 PM, allowing for a balanced work-life dynamic.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities, strengths, and limitations to establish individualized treatment plans.

• Implement therapeutic interventions using evidence-based practices tailored to each patient's specific conditions and goals.

• Collaborate with interdisciplinary healthcare teams to coordinate comprehensive care, ensuring ongoing communication about patient progress and adjustments to treatment plans.

• Document patient evaluations, treatment plans, and outcomes to maintain clear and concise records that facilitate appropriate billing and follow-up care.

• Monitor patient progress and adjust treatment protocols accordingly based on evaluations and interdisciplinary feedback.

• Participate in discharge planning and ensure that appropriate follow-up care is arranged for each patient, facilitating smooth transitions and optimal health outcomes.

• Stay abreast of relevant state regulations, corporate training, and compliance with all local, state, and federal guidelines concerning physical therapy practice.

• Engage in team meetings and contribute to initiatives aimed at improving patient access to care and services offered at our facility.

Patient Population / Client Focus

In this role, you will primarily work with patients facing various orthopedic challenges, including post-operative rehabilitation, sports injuries, and chronic pain conditions. Your clinical expertise will be critical in empowering individuals to reclaim their mobility and regain their quality of life. With a strong focus on individualized care, you will enhance the therapeutic experience for patients by addressing their unique needs and concerns.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program.

• Valid state licensure as a Physical Therapist (DPT).

• A minimum of two (2) years of outpatient orthopedic experience is essential.

• Current CPR certification to ensure patient safety in emergency situations.

Preferred Experience

• Experience with electronic health record (EHR) systems, enhancing efficient documentation and patient management.

• Prior work in a collaborative, interdisciplinary healthcare environment.
