Work Environment

As a Physical Therapist at Deaconess, you will work onsite at the patient's home, creating a comfortable and familiar setting for your therapy sessions. You will collaborate closely with a multidisciplinary team of healthcare providers, enhancing our patients' recovery experiences. This position entails a full-time schedule from Monday to Friday, allowing you to maintain a consistent work-life balance while dedicated to your patients' rehabilitation.

Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, including performing functional assessments and medical history reviews to tailor individualized treatment plans.
• Implement and execute treatment programs that may include therapeutic exercises, manual therapy techniques, and neuromuscular re-education to improve patient outcomes.
• Utilize various assessment tools and methodologies to track patient progress and adapt treatment methods as necessary.
• Provide education to patients and caregivers regarding treatment processes, preventive measures, and home exercise programs to promote independence.
• Demonstrate exemplary customer service by building rapport with patients and addressing their concerns during therapy sessions.
• Collaborate with the healthcare team to share insights and adapt care plans based on patient feedback and progress.
• Maintain accurate and timely documentation of patient interactions and treatment progress in compliance with organizational policies and healthcare regulations.

Patient Population / Client Focus

The patients you will serve include a diverse range of individuals, from orthopedic surgery recoveries to pediatric and geriatric patients requiring specialized care. Your treatments will span multiple realms, including sports medicine and neurological rehabilitation, allowing you to apply your expertise across various settings. Developing trust and connections with patients will be paramount in order to help them achieve their functional goals and improve their quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.
• Active state licensure as a Doctor of Physical Therapy (DPT) as mandated by the Physical Therapy Examining Committee.
• A minimum of two years of professional experience in a physical therapy setting, emphasizing home care is preferred.
• Current CPR certification, ensuring the ability to handle any emergencies during patient care.
• Strong verbal and written communication skills to effectively relay information to patients and other healthcare professionals.
• Proficient in word processing and basic computer skills to maintain documentation and communicate with the team.
• A personable demeanor with the ability to build and nurture strong relationships with patients and community members.

Preferred Experience

• Experience with electronic health records (EHR) systems and telehealth practices to enhance patient engagement.
• Background in patient education techniques and methods for improving patient adherence to therapy programs.

Compensation and Benefits
