Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate and address the emotional, social, and financial needs of patients and their families.

• Provide individual and group counseling focused on adjustment, grief, and coping mechanisms, tailored to the unique circumstances of patients facing end-of-life issues.

• Collaborate closely with interdisciplinary medical teams to create and implement tailored treatment plans that enhance patient care and address psychosocial support needs.

• Coordinate discharge plans in partnership with community organizations, ensuring continuity of care and appropriate post-discharge services.

• Engage in crisis intervention, providing immediate emotional support and counseling for patients and families in distress.

• Maintain accurate and timely documentation of patient encounters and care plans in compliance with hospital policies and legal requirements.

• Actively participate in hospital committees, sharing expertise on social work-related issues and contributing to policy development.

• Provide education to patients and their families regarding treatment options, community resources, and available support, ensuring they feel informed and empowered.

As a Licensed Clinical Social Worker in our hospice setting, you will primarily serve patients with terminal illnesses and their families. Compensation and Benefits\n

The position offers a competitive salary range of USD 70,000 – 100,000 annually, commensurate with experience and qualifications.
