Key Responsibilities\n

• Engage with program participants through telehealth sessions, using a client-centered approach to help them set and achieve their mental health goals.

• Proactively work with participants to prevent crises and reduce the need for emergency services through early intervention and systematic support.

• Maintain detailed documentation of crisis alerts in coordination with county crisis services, to enhance the continuum of care.
\n
• Oversee transitions for clients moving to and from hospital or inpatient services, collaborating closely with hospital staff to ensure smooth care coordination and discharge planning.

• Develop personalized treatment plans in partnership with clients, regularly assessing progress and adjusting services as needed to support their evolving goals.

• Perform additional duties as required to enhance the quality of service provided, ensuring that the needs of clients are met in a high-volume caseload setting.

Qualifications\n

• Master's degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

• Avalid and active license as either an LMFT, LPC, or LCSW in Oregon.

Compensation and Benefits\n

This position offers a competitive salary ranging from $100,000 to $105,000 per year , commensurate with experience and caseload responsibilities.
