Work Environment

The Home Healthcare Physical Therapist will operate primarily in patients' homes across the Hillsborough area. This unique work setting allows you to foster close relationships with your patients and their families, which is key to effective therapy. You will work autonomously while having the support of our professional team for any challenging cases that arise. Your day will frequently involve traveling between patient homes, requiring efficient time management and organization to maximize efficacy during visits.

Key Responsibilities

• Perform comprehensive assessments of patients' physical capabilities, limitations, and therapeutic needs through methods such as physical evaluations, interviews, and clinical testing.
• Develop individualized care plans that reflect the unique goals and circumstances of each patient, leveraging your clinical expertise to ensure optimal therapeutic outcomes.
• Implement hands-on therapy techniques tailored to patient needs, including therapeutic exercises, manual therapy, gait training, and balance enhancement activities.
• Educate patients and their families on recovery processes, pain management strategies, and exercises to continue improving their conditions at home.
• Maintain meticulous and timely documentation of patient evaluations, treatment plans, and progress notes, aligning with company policies and insurance standards.
• Monitor patient progress consistently, adapting treatment plans as necessary based on patient response and progress, and communicate effectively with both the family and supervising physician.
• Ensure compliance with regulatory guidelines, providing safe and effective clinical practices throughout all physical therapy sessions.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including individuals recovering from surgery, those managing chronic conditions, elderly clients experiencing age-related mobility issues, and patients with neurological conditions. Your expertise in customizing therapy programs will be pivotal in helping these patients regain mobility, strength, and confidence in their daily activities.

Qualifications

• A Doctoral or Master's Degree in Physical Therapy from an accredited institution.
• A valid and active Physical Therapy license in the state of New Jersey.
• A minimum of 2 years of professional experience in physical therapy, particularly within homecare or outpatient settings.
• Strong clinically sound assessment skills and a breadth of experience in various manual therapy techniques.
• Certifications in CPR and First Aid are preferred; additional certifications in geriatrics, neurological rehabilitation, or other specialized fields are welcomed.
• A valid driver's license and reliable transportation are required to facilitate travel to patients' residences.

Preferred Experience

• Experience in home health care settings to manage logistical challenges and deliver care effectively in non-traditional environments.
• Demonstrated ability to communicate effectively with patients and families, ensuring they are engaged in their recovery journey.
• Familiarity with electronic health record systems to seamlessly document patient information and therapy progress.

Compensation and Benefits
