This role is fully remote, providing flexibility while still ensuring that our LCSWs remain integral to the Hospice Center team. Although remote, there may be occasions that require on-site presence at our medical center to address urgent psychosocial matters directly.

Key Responsibilities\n

• Independently manage a targeted caseload of hospice patients, conducting thorough evaluations to determine social, emotional, and cultural factors influencing their health and treatment plans.

• Lead interdisciplinary team meetings, coordinating efforts to address life care decisions, advance directives, and crisis intervention for high-risk patients.

• Develop and implement comprehensive treatment plans while collaborating closely with case managers to ensure the psychosocial and socioeconomic needs of patients and families are met.

• Provide education and ongoing support to healthcare team members about the pivotal role of social work in patient care and the nuances of psychosocial issues.

• Supervise and mentor other social workers, MSW students, and interns, guiding them towards best practices in patient engagement and care delivery.

• Facilitate connections with community resources and local agencies to prevent hospital readmission, ensuring that patients have the necessary supports in place post-discharge.

• Respond to psychosocial service requests in a timely manner, ensuring there are no delays in treatment implementation or discharge timelines.

• Participate in on-call rotations on a regular basis, ready to provide support during unexpected situations.

• Engage actively in patient care conferences and specialty team rounds to discuss specific cases and enhance collaborative care strategies.

• Plan and coordinate care for patients, especially children, that requires collaboration with external community agencies to monitor ongoing clinical needs.

Qualifications\n

Compensation and Benefits\n

This position offers a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
