Key Responsibilities\n

• Provide clinical consultation and support to fellow staff members regarding mental health issues affecting diverse patient demographics, including geriatric patients and individuals dealing with substance use disorders.

• Effectively communicate critical patient information, including social histories, to ensure continuity of care among team members and external providers.
\n
• Develop, coordinate, and recommend personalized treatment plans based on individual patient needs, collaborating effectively with both patients and their families regarding care decisions, especially in relation to home environments.

• Conduct regular case reviews and treatment planning sessions, leveraging evidence-based practices to assess the effectiveness of current interventions.

• Document patient encounters and progress notes promptly, ensuring compliance with organizational policies and standards.

• Perform additional responsibilities as assigned, contributing to the overall effectiveness of the counseling department.

Patient Population / Client Focus\n

Your primary focus will involve engaging with a varied patient population, which includes adolescents, adults, and older adults facing different mental health challenges. You will specialize in tailoring your counseling approaches to meet the needs of clients with complex backgrounds, ensuring a compassionate and responsive approach to their care.

Qualifications\n

• A Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is a mandatory requirement for this position.

• Active independent licensure is required, such as LPCC, LCSW, or LMFT.

• Exceptional skills in documentation and progress note completion after client sessions to ensure accurate and timely record-keeping.

Preferred Experience\n

• A minimum of two years of experience working in a clinical mental health setting is preferred, with a strong emphasis on geriatric care and substance use disorders.
