Key Responsibilities\n

• Develop individualized treatment plans tailored to the unique needs of each client, based on thorough clinical assessments.

• Actively participate in daily huddles and team meetings as part of a multidisciplinary approach to care, contributing insights and updates on your clients' progress.

• Monitor and adjust treatment plans as necessary, following up with clients to ensure they are experiencing improvements in their situation.

• Engage with clients’ families to provide support and information on coping strategies, enhancing their involvement in the caregiving process.

• Collaborate with educational institutions, when applicable, to provide resources and support for students facing challenges, ensuring their needs are met holistically.

Patient Population / Client Focus\n

As a Senior Clinical Social Worker at our hospice center, you will primarily work with terminally ill patients and their families, providing counseling and support during one of the most challenging times in their lives. Your expertise will help clients navigate emotional and psychological challenges, ensuring dignity and comfort. By focusing on holistic care, you will support both patients and their families in coping with the complexities of end-of-life care.

Qualifications\n

• A current Licensed Independent Clinical Social Worker (LICSW) license.

• A Master’s degree in Social Work from an accredited program.
