Work Environment\n

This remote work position offers flexibility and support for our licensed clinical social workers. You will operate from the comfort of your home while being closely integrated into our hospice team. Regular meetings and virtual check-ins will be scheduled to uphold a collaborative work environment, ensuring that our social workers feel connected and supported in their roles. With a structured Monday to Friday workweek from 9:00 AM to 5:00 PM, you will maintain a healthy work-life balance while delivering essential services to our clients.

Key Responsibilities\n

• Conduct age and culturally appropriate comprehensive biopsychosocial assessments to address mental health, substance abuse, and medical needs within the hospice care framework.

• Provide a range of behavioral health services, including individual and group counseling, intake evaluations, and treatment planning tailored to each patient's unique situation.

• Offer crisis intervention and make necessary referrals to community resources, ensuring that patients and families have access to comprehensive care, including medication assistance programs.

• Document mental health evaluations, progress notes, and suicide risk assessments in compliance with electronic health record systems to ensure accurate patient tracking and collaboration with other healthcare providers.

• Engage in Quality Improvement initiatives to enhance services based on data collection and analysis, fostering continuous improvement in patient care.

Patient Population / Client Focus\n

The main focus of this role will be individuals and families within a hospice setting, often dealing with complex emotional challenges related to terminal illness. You will work closely with patients, caregivers, and interdisciplinary teams to provide empathetic support tailored to the unique circumstances surrounding end-of-life care. Qualifications\n

• Valid and current Licensed Clinical Social Worker (LCSW) license in the state of California.

Compensation and Benefits\n
