Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients’ mental, emotional, and social needs in the hospice setting, ensuring that their concerns and strengths are comprehensively understood.

• Develop and implement tailored treatment plans that target clients’ unique goals and preferences, utilizing evidence-based therapeutic modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-focused interventions.

• Facilitate individual, family, and group therapy sessions, delivering high-quality therapeutic support that emphasizes comfort and emotional healing.

• Collaborate effectively with the healthcare team to coordinate care, providing updates and insights into clients' progress during regular case management meetings.

• Offer immediate crisis intervention and risk assessment to ensure client safety, developing safety plans as required.

• Maintain accurate and up-to-date documentation of client interactions in compliance with ethical and legal guidelines, supporting the continuity and effectiveness of care.

• Lead group therapy sessions and psychoeducation classes to educate clients and their families about mental health and coping strategies.

Patient Population / Client Focus\n

Your work will primarily involve clients affected by terminal illness or those in hospice care, as well as their families. You will address a wide array of issues, including grief, anxiety, and depression related to end-of-life experiences. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Possession of an active state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

This full-time role offers a competitive salary range of $80,000 to $100,000 annually, supplemented by a sign-on bonus.
