Key Responsibilities\n

The Licensed Professional Counselor will be responsible for:

• Conducting thorough assessments and evaluations to identify the needs of clients and create personalized treatment plans.

• Facilitating open communication with supervisors, team members, and client families to provide progress updates and coordinate care.

• Tracking and sharing treatment progress through regular documentation, including a tentative schedule of appointments, home visits, and observation sessions.

• Conducting follow-up interviews, home visits, and observations to ensure that clients' ongoing needs are met effectively.

• Participating in interdisciplinary team meetings to discuss client progress and treatment strategies.

• Preparing monthly and quarterly reports that detail progress in client treatment plans.

• Arranging and leading training sessions for team members on program implementation and documentation best practices.

• Managing client records diligently to ensure all information is organized and accessible to relevant staff.

• Acting as a liaison between clients and the mental health community, helping clients achieve their personal development goals.
