Work Environment\n

This role is firmly rooted in a remote work setting, allowing for flexible engagement with clients, primarily focused on school-aged populations. As a member of our team, you will have the opportunity to operate within a variety of environments, ranging from home visits to virtual consultations, facilitating a seamless connection with clients and their families. The standard working hours are Monday to Friday from 8:00 AM to 4:30 PM, though occasional evening and weekend availability may be required to meet the needs of our clients. Key Responsibilities\n

• Conduct comprehensive assessments to identify the emotional, mental, and social needs of students, leveraging your expertise to guide interventions.

• Collaborate closely with students, their families, and educational staff to create personalized care plans that prioritize individual goals and circumstances.

• Provide counseling and emotional support to students and their families, assisting them in navigating pivotal life transitions such as grief, family changes, and educational pressures.

• Serve as an advocate for clients, ensuring their rights are upheld and they gain access to necessary community resources and support services.

• Work in partnership with interdisciplinary teams, including educators, healthcare professionals, and family members, to ensure a cohesive approach to care and support.

• Conduct regular follow-ups with clients to monitor their well-being, assess their living conditions, and address safety and health concerns.

• Facilitate effective communication among family members and educational staff, resolving conflicts and coordinating care strategies.

• Provide educational resources to clients and families regarding local community programs, support groups, and services that can enhance social engagement.

• Maintain meticulous documentation of assessments, care plans, and progress notes within electronic health record (EHR) systems to support continuity of care and compliance with professional standards.

• Stay informed of relevant social work practices, policies, and regulations to ensure ethical compliance in all client interactions.

Your primary focus will be on supporting school-aged clients and their families. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
