The successful candidate will be responsible for conducting thorough assessments, developing treatment plans, and delivering social work services within our Community Health Clinic.

This position is primarily remote, which allows you to effectively balance your professional commitments with personal needs. You will be part of a collaborative Zoom-based team environment, engaging with medical staff and other social workers to address patients' needs while utilizing various digital platforms for case management and documentation.

Key Responsibilities\n

• Psychosocial Assessment: Conduct timely psychosocial evaluations of patients, utilizing your theoretical knowledge of human behavior and understanding of the psychosocial factors influencing treatment outcomes. Formulate ethical considerations, especially regarding mandatory reporting and patient rights.

• Clinical Intervention: Provide supportive counseling to patients and their families, facilitating family conferences, and helping navigate challenging decisions regarding care options such as hospice or DNR. Collaborate closely with Case Managers for efficient discharge planning.
\n
• Caseload Management: Skillfully manage a clinical caseload by identifying high-risk situations, optimizing referrals, and balancing ongoing patient care. Seek assistance from colleagues and supervisors when necessary.
\n
• Documentation and Legal Standards: Ensure comprehensive documentation of all interventions, cases, and care plans per the organizational policies. Adhere to all legal ethical standards, including issues of suspected abuse or neglect and the Tarasoff duty to warn.
\n
• Training and In-Service: Provide in-service trainings for hospital staff on the psychosocial aspects of patient care, contributing valuable insights during Lunch and Learns.
\n
• Quality Improvement Initiatives: Participate in quality improvement projects that analyze and monitor patient care processes and outcomes, contributing to a culture of continuous enhancement.
\n
• Supervision: Oversee and mentor graduate master-level students and MSW employees assigned to your team, fostering professional growth and clinical skill development.
\n
• Community Engagement: Actively contribute to Social Work Month activities and lead cancer-specific support groups, enhancing community engagement and support structures for patients and families.

Patient Population / Client Focus\n

The LCSW will work with a diverse patient population, including individuals facing complex psychosocial challenges related to chronic illness, particularly cancer. Engagement with families is critical, as they often require support during some of the most challenging decisions in life. Your interventions will be instrumental in improving patient outcomes through enhanced emotional and psychological support.

Qualifications\n

• Master's degree in Social Work from an accredited school.

• 2 years of experience in a medical setting, with at least 1 year in an oncology-focused role.
\n
• Possess a valid California Clinical Social Worker License (LCSW).
\n
• Basic Life Support (BLS) certification from the American Heart Association.
\n
• Excellent interpersonal, verbal, and written communication skills, with a demonstrated ability to work effectively in a team-oriented environment.

Compensation and Benefits\n
