Work Environment

This role is primarily onsite, providing services in the homes of patients throughout Sarasota and the surrounding areas. You will work in varied home environments, which will require adaptability and resourcefulness. Key Responsibilities

• Direct physical therapy treatment designed to prevent disability, relieve pain, and restore muscle function.
• Evaluate patients' physical abilities through diagnostic tests and create individualized therapy plans based on these assessments.
• Supervise and guide physical therapy assistants to ensure effective delivery of care as per the prescribed treatment plan.
• Assess home environments to recommend modifications or support systems that promote independence and rehabilitation.
• Administer various interventions, including manual therapy techniques, therapeutic exercises, and use of physical agents like traction and massage.
• Fit and adjust prosthetic and orthotic devices, advising physicians on necessary modifications.
• Maintain accurate clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with OASIS and agency policies.
• Collaborate with an interdisciplinary team to discuss case progress and adapt treatment plans, ensuring holistic patient care.
• Consult with patients and families on effective therapy practices and educate them on the use of therapeutic devices.
• Participate in ongoing training sessions to stay updated on best practices and emerging trends in physical therapy.

As a Home Care Physical Therapist, you will work with a diverse patient population, including the elderly, individuals recovering from surgery, and those with chronic health conditions. Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Current and valid physical therapy license in the state of Florida.
• CPR certification and valid driver's license with automobile insurance.
• Strong communication skills, with an emphasis on interpersonal interactions and family engagement.
• Ability to advocate for patients and support them through various phases of their recovery.
• Proficient understanding of federal, state, and governing agency regulations impacting home healthcare.
• Creative problem-solving skills and a positive attitude towards collaborative patient care.

Preferred Experience

• A minimum of two years of experience as a physical therapist, preferably in home care or acute care settings.

Compensation and Benefits

We are proud to offer a competitive salary ranging from USD 100,000 to 120,000 annually, along with a comprehensive benefits package.
