Work Environment

This role involves working onsite in clients' homes, offering the advantage of providing personalized and direct therapy in the comfort of their own environment. This position also allows for flexible scheduling, including standard hours from Monday to Friday or an option for four 10-hour shifts, allowing for a rewarding work-life balance.

Key Responsibilities

• Conduct comprehensive assessments to evaluate patient mobility, strength, and functional abilities, identifying any movement limitations.
• Develop, implement, and adjust personalized rehabilitation plans based on patient assessments, ensuring that each plan aligns with individual recovery goals.
• Deliver hands-on therapy, helping clients perform therapeutic exercises and practice functional tasks that will aid in their recovery.
• Collaborate with a multidisciplinary team—including physicians, occupational therapists, and other health professionals—to coordinate comprehensive care for patients.
• Educate patients and their families on appropriate home exercises, adaptive techniques, and how to track progress effectively.
• Document all patient interactions and treatments accurately in compliance with legal and ethical standards to ensure continuity of care.

Patient Population / Client Focus

You will be working primarily with patients recovering from surgeries, managing chronic conditions, or overcoming injuries that impact their mobility. Our clients range from older adults seeking to regain independence after joint replacements to younger patients working to recover from sports injuries. Your holistic approach will play a crucial role in their rehabilitation journey.

Qualifications

• Bachelor's Degree in Physical Therapy or a related field is required, with eligibility for or possession of an active Florida Physical Therapy License.
• Exceptional clinical decision-making and communication skills to effectively interact with clients, their families, and interdisciplinary health professionals.
• A commitment to ongoing professional development, staying current with evidence-based practices in physical therapy.

Preferred Experience

• A minimum of 2 years of experience in a similar physical therapy role is preferred, particularly in home health or outpatient settings.
• Experience in using electronic health record (EHR) systems for documentation and patient management.
• Familiarity with evidence-based physical therapy practices and a variety of therapeutic modalities.

Compensation and Benefits
