Key Responsibilities\n

• Utilize your clinical training and experience to conduct thorough psychosocial assessments and form independent diagnoses for patients in a hospice care setting.

• Work in partnership with patients and their families to cultivate patient-focused treatment plans that reflect their personal goals and circumstances.

• Implement evidence-based therapeutic interventions tailored to individual preferences and needs, such as solution-focused therapy, motivational interviewing, and behavioral activation, ensuring robust support for emotional and psychological health.

• Monitor and document patient progress systematically through regular evaluations, ensuring timely adjustments to treatment as needed, either in-person or via telehealth platforms.

• Assist in the management of psychotropic medications as indicated, focusing on monitoring adherence, addressing side-effects, and evaluating the efficacy of prescribed treatments.

Patient Population / Client Focus\n

This role chiefly involves working with patients in hospice care who are navigating complex psychological and emotional challenges related to terminal diagnoses. Qualifications\n

• A Master’s degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).

• A current, valid, and unrestricted LCSW license to practice independently in New York State.

• Minimum of one year of post-licensure experience as a therapist in a mental health setting, preferably with end-of-life or hospice care experience.

Compensation and Benefits\n

Our competitive salary range for this position is between $80,000 and $100,000 annually, commensurate with experience and education. In addition to a base salary, eligible candidates will receive a sign-on bonus and a comprehensive benefits package that includes:
