You will work primarily in the home settings of our patients, providing services in a familiar and comfortable environment. Key Responsibilities

• Manage clinical outcomes effectively by addressing outlier cases with your team of staff therapists, ensuring consistent and high-quality service delivery.
• Streamline clinical workflows to enhance patient engagement by improving the processes for patient capture, referral management, and scheduling.
• Develop and execute personalized therapy plans focused on early intervention and same-day evaluations, closely monitoring patient progress and making necessary adjustments.
• Foster a collaborative working environment by encouraging participation from all colleagues, promoting teamwork, quality care, and adherence to patient safety standards.
• Work with the Director of Therapy Operations (DTO) to identify opportunities for clinical improvements, setting benchmarks for service excellence.
• Build and maintain relationships with employers, referral sources, and payer groups, ensuring consistent communication and engagement to enhance service delivery.
• Utilize clinical systems for comprehensive documentation, optimal coding, and thorough follow-through on all patient care orders.
• Adhere to regulations defined by the Florida State Practice Act, ensuring compliance throughout your practice.
• Drive metrics related to patient and client satisfaction, consistently aiming to exceed patient expectations.
• Exemplify all aspects outlined in the Physical Therapy Guidebook, ensuring each patient visit adheres to established best practices.

Patient Population / Client Focus

Your primary focus will be on diverse patient populations, ranging from those recovering from surgical procedures to individuals managing chronic conditions. You will deliver tailor-made therapy that reflects each patient's unique needs and rehabilitation goals, ultimately contributing to improved health outcomes and enhanced overall well-being.

Qualifications

• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited academic institution.
• Current and active licensure as a Physical Therapist in the state of Florida, demonstrating your commitment to upholding the standards of practice.
• Exceptional problem-solving abilities, using logical reasoning to identify key issues, explore potential solutions, and implement effective changes.
• Outstanding customer service skills, characterized by the ability to interact with patients and colleagues tactfully and professionally.

Preferred Experience

• Prior experience in home health care or outpatient therapy settings is highly desired, though new graduates with strong clinical training are encouraged to apply.
• Familiarity with clinical documentation systems and adherence to coding regulations will provide an advantage in managing patient information efficiently.
• A background in collaborating with multidisciplinary teams to enhance patient care strategies is preferred.

Compensation and Benefits

• Annual salary ranging from USD 120,000 to 130,000, commensurate with experience.
