Work Environment

This position is onsite and integrates into the home care environment, allowing our therapists to work directly with patients in their homes. You will be expected to travel across the Miami area to deliver services, providing a flexible and dynamic work setting. Key Responsibilities

• Conduct comprehensive assessments of patients' physical capabilities and rehabilitation needs upon intake to determine appropriate therapy plans.
• Administer individualized therapy sessions aimed at returning patients to their optimal functional status, including utilizing therapeutic exercises, manual therapy techniques, and patient education.
• Collaborate with healthcare professionals and support staff to manage clinical outcomes, including addressing unique outlier cases as they present.
• Optimize patient workflows by improving referral processing and scheduling to maximize therapy throughput without compromising patient care quality.
• Deliver timely interventions—including same-day evaluations—to ensure immediate support when patients are most vulnerable.
• Engage in ongoing communication with external stakeholders, including referral sources, employers, and payer groups, to develop productive relationships that enhance clinic visibility.
• Participate in team meetings to exchange knowledge, review clinical improvements, and maintain quality control in service delivery.

Patient Population / Client Focus

Your work will primarily focus on a diverse patient population ranging from those recovering from surgeries and injuries to individuals managing chronic conditions or disabilities. A significant emphasis will be placed on developing tailored rehabilitation strategies that accommodate the unique circumstances of each patient, enhancing their overall mobility and functional independence.

Qualifications

• Active State licensure in Florida as a Physical Therapist is mandatory.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
• Demonstrated excellent communication skills alongside strong interpersonal abilities essential for building rapport with patients and team members.
• Superb organizational capabilities to manage multiple cases and appointments efficiently in a fast-paced home care environment.

Preferred Experience

• Experience in home care settings would be valuable, particularly with patient rehabilitation and mobility training.
• Understanding of the challenges faced by patients recovering at home, including environmental considerations and family dynamics.
• Familiarity with electronic health record (EHR) systems for accurate documentation and communication within the healthcare team.

Compensation and Benefits

This full-time position offers a competitive salary ranging from $120,000 to $130,000 per year, commensurate with experience and caseload.
