Work Environment

This role is predominantly onsite in the homes of patients in Miami, allowing for a unique perspective on client care and rehabilitation needs. Working out of patients' residences provides physical therapists with the opportunity to evaluate and adapt therapy effectively to real-life environments. Key Responsibilities

• Direct and implement physical therapy treatment plans that prevent disability, alleviate pain, and restore or enhance muscle function as per each patient's specific needs.
• Review and supervise the work of physical therapy assistants to ensure they are following the prescribed care plans accurately.
• Conduct thorough assessments of clients' functions using diagnostic tests and interpret findings to develop effective treatment strategies.
• Analyze and modify patients' home environments, suggesting adaptive techniques and tools to promote greater independence in daily activities.
• Deliver physical therapy interventions as directed by a physician, ensuring that all techniques align with best practices.
• Administer manual therapeutic exercises and motivate clients during non-manual exercises, guiding them toward functional recovery.
• Evaluate, fit, and provide adjustments to orthotic and prosthetic devices, consulting with physicians for necessary modifications.
• Prepare and maintain accurate clinical documentation, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with all health regulations.
• Engage as a valued member of the interdisciplinary care team, contributing to case conferences and total treatment planning for clients.
• Educate clients and their families on physical therapy techniques, device usage, and the overall rehabilitation process.
• Conduct on-site functional assessments and collaborate on the revision of care plans as needed to meet changing client needs.
• Maintain timely communication and documentation in accordance with company policies and departmental requirements.

Patient Population / Client Focus

The ideal candidate will work with a diverse range of clients, primarily focusing on those requiring orthopedic rehabilitation. This could include post-surgery patients, individuals recovering from injuries, or those managing chronic pain. By delivering tailored therapy in the comfort of their homes, the physical therapist will play a crucial role in enhancing their quality of life and promoting optimal function and independence.

Qualifications

• A graduate degree from an accredited physical therapy program.
• A minimum of one year of practical experience in a physical therapy role, preferably in an acute care or home care setting.
• Current PT licensure in Florida and CPR certification.
• A valid driver's license and automobile insurance to facilitate travel to client homes.
• A strong commitment to integrity, quality, and clinical excellence.
• Excellent interpersonal skills with a positive attitude to work effectively within a collaborative team environment.

Preferred Experience

• Proven experience in home health care settings is a plus, particularly with orthopedic patients.
• Familiarity with documentation systems such as OASIS and electronic health records.
