This role operates within a community health clinic setting, allowing licensed counselors to serve a diverse client population from the comfort of their own homes. Key Responsibilities\n

• Conduct thorough assessments and evaluations to understand client needs, developing tailored treatment plans based on individual circumstances.

• Maintain open communication with supervisors and interdisciplinary team members to discuss client progress, adapting strategies as necessary to ensure effective treatment.

• Coordinate and schedule home visits, ensuring compliance with in-home observation requirements, conducting chart reviews, and performing data analysis to enhance client support.

• Ensure the efficacy and execution of treatment plans, actively participating in team meetings to share insights and progress updates.

• Document client interactions and progress meticulously through monthly and quarterly reports, maintaining accurate records that are accessible to authorized personnel.

• Serve as a key liaison between clients, family members, and the broader mental health community, promoting resources and support to meet developmental goals.

• Organize and facilitate training sessions to certify that staff are competent in implementing program plans and documentation processes.

• Provide ongoing follow-up support through interviews, home visits, and observations to guarantee the continuity of care for clients.

The LPC in this position will work with a diverse clientele, which includes adults and youth facing various mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is required.

Compensation and Benefits\n
